| 2008 Preventive Maintenance / Audit Program

U.S. Bottlers Machinery Company will provide our factory trained technician to visit your facility on a conveniently flexible schedule and remain in effect for a period of one (1) year after the first appointment.

If Quarterly or higher is selected, you will receive the following:
- A U.S. Bottlers Machinery Company Preventive maintenance / Audit Checklist customized to each machine under contract.
- Our Service Technician will work with or without personnel to evaluate machinery, perform preventive maintenance/Audit. provide training, and recommend machine enhancements. This program is focused on increasing/efficiencies and reducing down time.
- Installations, machine overhauls, rebuilds, and other major repairs and emergencies are not covered under this agreement. However, you will receive Priority Service over non-plan holders.
- Under this agreement the Technician's time will be billed at a rate of $75 per hour, Monday through Friday. $112.50 per hour for overtime and Saturday, and $150 per hour on Sunday. If a technician is requested to work over a holiday, a special $276 per hour rate will apply. Travel and living expenses will be billed at actual cost.
- All parts not covered under a manufacturer's warrenty that are ordered from U.S. Bottlers Machinery Company will be charged at our prevailing list price, less a 5% discount for the duration of this agreement.
- Customer will be billed upon service, under a PO number supplied by the customer
